We are excited to welcome you to our clinic! To help make your first visit smooth and productive, here are a few things you can do to prepare:

 1. Gather Your Medical Records

Bring copies of:

  • Most Recent lab tests, imaging (X-rays, MRIs, CT scans)

  • Prior rheumatology or specialist notes(Last 2 visits)

  • Most recent Hospital discharge summaries, if any

  • List of all current medications
     

2. Bring a Complete Medication List

Include:

  • Prescription medications

  • Over-the-counter drugs

  • Vitamins and supplements

  • Any allergies or past side effects


3. List Your Symptoms

Write down:

  • When your symptoms started

  • What makes them better or worse

  • How they impact your daily life


4. Bring Insurance and ID

  • Government-issued photo ID

  • Insurance card(s)

  • Referral from your primary care doctor, if required by your insurance
     

5. Complete New Patient Forms

You can download and fill out the new patient forms from our website, or arrive 15–20 minutes early to fill them out in the office.


6. Wear Comfortable Clothing

If you’re having joint pain, wear clothes that allow for easy examination of areas like knees, elbows, or shoulders.

At Elite Rheumatology and Arthritis Center, your health and comfort are our top priorities. Here's what you can expect during your first appointment with Dr. Sumalatha Nandikonda, board-certified rheumatologist:

1. Comprehensive Evaluation

Dr. Nandikonda will take time to:

  • Review your full medical history

  • Ask detailed questions about your symptoms, pain, stiffness, fatigue, or swelling

  • Understand your lifestyle and how your condition is affecting daily activities

 

2. Physical Examination

She will carefully examine your:

  • Joints, muscles, skin, and range of motion

  • Areas of pain, swelling, or tenderness

  • Other signs of autoimmune or inflammatory conditions

 

3. Review of Labs and Imaging

If you’ve brought prior lab results or imaging, Dr. Nandikonda will go over them.
She may also order:

  • Additional blood work

  • X-rays, ultrasounds, or MRIs if needed

  • Specialized tests to help diagnose autoimmune diseases

 

4. Diagnosis and Education

Once the evaluation is complete, she will:

  • Discuss possible diagnoses

  • Explain your condition in simple terms

  • Answer your questions with compassion and clarity

 

5. Personalized Treatment Plan

You will receive a custom plan which may include:

  • Prescription medications or biologics

  • Lifestyle or dietary recommendations

  • Injections, physical therapy, or referrals for further care

 

6. Follow-Up and Support

  • You may be scheduled for follow-up visits to monitor progress

  • eam is available to help coordinate labs, imaging, and prior authorizations

Patient care: Prescription Refills:

All prescription refills should be called into the office during the normal business hours. Please allow 48 hours for your request to be reviewed. Refill request are not taken after 3:00 p.m. on Fridays or during weekends or holidays. Please give the office 48 hours on all prescription request.

Fees & Payments:

We make every effort to keep down the cost of your medical care. Our office policy is to collect patients payment responsibilities at the time of visit.

  • Medication Refill Instructions – Elite Rheumatology and Arthritis Center

    ​

    Dear Patient,

    To make sure you receive your medications on time, please follow these steps:

    ​

    1. Check with Your Pharmacy First

      • Contact your pharmacy to see if you have any refills left on your prescription.
         

    2. If No Refills Remain

      • Ask your pharmacy to send us an electronic refill request.

      • Please allow 48 hours for our office to process your request.
         

    3. If Your Refill Is Delayed

      • Send Dr. Nandikonda a secure message through your online patient portal. This is the best way of communication for prompt reponse. You should get a response from us in 24 hrs (except weekends).

      • If no reponse, Please call our clinic and let my staff know.
         

    4. Possible Insurance Delays

      • Sometimes, your insurance company may require a prior authorization (PA) before approving coverage.

      • This process can cause delays, but our staff will work with you to complete the PA as quickly as possible.

      • You may also contact your insurance company directly to ask:

        • Why the medication is not covered, and

        • What alternatives in the same class are on your formulary.
           

    5. How You Can Help

      • If possible, obtain your insurance formulary list and share it with us.

      • This will help us prescribe medications that are covered under your plan.

         

    We understand how stressful it can be to run out of medication. Please know that my team and I are committed to working with you and your pharmacy to ensure you receive the best rheumatology care in a timely manner. We strongly encourage you to register and use our online patient portal for your medical care.

    We offer affordable rheumatology care with transparent pricing:

    • New Patient Rheumatology Consultation: $150 (flat rate)

    • Follow-Up Rheumatology Visits: $100 (flat rate)

    • Procedure Charges: Vary depending on the procedure

    Any changes to the fee schedule will be communicated to you prior to your visit.
